These terms apply to your use of the At-Home Health website. By using the site, you agree to them. If you do not agree, please do not use the site.

General Information Only

Everything published here, including service descriptions, answers to frequently asked questions, and general explanations of the Medicare home health benefit, is general information.

It is not medical advice, diagnosis, or treatment, and it is not advice about your individual coverage. Your own situation should be discussed with your physician or other authorized healthcare provider.

No Patient-Provider Relationship

Visiting this website, reading its content, or submitting a form does not create a patient-provider relationship with At-Home Health and does not begin services.

How Services Actually Begin

Home health services are not requested and granted through this website. Services depend on all of the following:

• A clinical assessment of the patient's needs
• An order from a physician or other allowed healthcare practitioner, where applicable
• A review of eligibility and medical necessity
• Coverage requirements applicable to the patient
• Availability of qualified staff, and confirmation that the patient's location is within our approved service area
• An accepted and established individualized plan of care

At-Home Health reviews each referral individually and does not accept every referral.

Coverage Is Not Guaranteed

Nothing on this website guarantees Medicare coverage, insurance coverage, eligibility, admission, service approval, a visit schedule, a start date, or a clinical outcome. Benefits and eligibility vary from person to person and must be verified individually, and plan participation can change.

Please call us at (310) 757-2037 before services begin so that coverage can be reviewed for the specific patient. Do not rely only on the name printed on an insurance card.

Automated Calls

If you give separate, optional permission when you submit an inquiry, we may contact you using an automated calling system, including a call that uses an artificial or AI-generated voice. These calls exist to respond to your inquiry and to gather basic, non-clinical information. They are not medical advice, not a clinical assessment, and not a decision about eligibility, coverage, services or admission. Permission is not a condition of receiving services, and you may withdraw it at any time. Calls may be recorded and transcribed, and we will say so at the start of the call.

Text Messages

If you tick the optional consent box when you submit the Care Options form, we may send text messages to the mobile number you gave us about that request. Today that means a short message confirming we received your inquiry and that a member of our care team will call you. We may also text you about the same request if we cannot reach you by telephone.

Consent to text messages is optional and is never a condition of receiving care. If you do not give it, we will still respond to your inquiry by telephone.

You must be at least 18 years old to enroll in our SMS messaging program. This age requirement applies only to enrollment in text messaging. It does not affect eligibility for care, and it does not limit who may contact us or submit the Care Options form.

Message frequency varies. Message and data rates may apply; those are charged by your mobile carrier, not by At-Home Health. Reply STOP to any message to stop receiving texts, or HELP for help. You may also withdraw consent by contacting our office. Carriers are not liable for delayed or undelivered messages.

Text messages are not monitored for emergencies. If you are experiencing a medical emergency, call 911.

Information May Change

Content on this website may be updated, corrected, or removed at any time without notice. We make a reasonable effort to keep it accurate, but we do not warrant that every page is complete or current at the moment you read it.

The text, layout, design, photographs, illustrations, and code on this website are owned by At-Home Health or used with permission from their licensors, and are protected by copyright and other laws.

These terms are governed by the laws of the State of California, without regard to its conflict-of-law rules. If any provision is found unenforceable, the remaining provisions continue to apply.

Changes to These Terms

We may revise these terms as the website changes. When we do, the last-updated date at the top of this page will change, and continuing to use the site means the revised terms apply.
